Who is eligible?
A person receiving county board services must be a resident of Licking County who has been diagnosed with a developmental disability or delay before the age of 22.
How do I make a referral?
Birth to age 3
Anyone can make a referral to Licking County’s Early Intervention program. You will need the child’s name, date of birth, address, parent’s name and contact information. To make a referral, please call 1-800-755-4769. You can also make a secure online referral at http://bit.ly/ReferToHMG .
Age 3 and older

What are the qualifications?
Children birth to age 3
Children who have at least one developmental delay or an established risk for a developmental delay are eligible for our Early Childhood services.
Youth age 3 to 21
Children age 3 to 6 who have at least two developmental delays or an established risk are eligible to receive services provided by, or paid for, by our board.
Eligibility for children between the ages of 6 to 15 occurs when there is a developmental disability and substantial functional limitations in at least three of the following areas of major life activity (self-care; receptive and expressive language; learning; mobility; self-direction; and capacity for independent living). LCBDD uses the Children’s Ohio Eligibility Determination Instrument (COEDI) for determining eligibility for this age group.
Youth age 16 and older are eligible when there is a developmental disability and they have substantial functional limitations in at least three areas of major life activity (the capacity for economic self-sufficiency; self-care; receptive and expressive language; learning; mobility; self-direction and capacity for independent living). LCBDD uses the Ohio Eligibility Determination Instrument (OEDI) for determining eligibility for this age group.
Adults age 22 and older
Adults age 22 and older who have a developmental disability and substantial functional limitations in at least three of the following areas of major life activity (the capacity for economic self-sufficiency; self-care; receptive and expressive language; learning; mobility; self-direction and capacity for independent living) are eligible to receive board services or support. LCBDD uses the Ohio Eligibility Determination Instrument (OEDI) for determining eligibility for this age group.
It is important to note that developmental disabilities must be assessed by a licensed psychologist and/or physician. Additionally, functional limitations are evaluated through completion of a COEDI or OEDI. Families must also submit documentation of a qualifying diagnosis from a medical doctor, doctor of osteopathic medicine, certified nurse practitioner, doctor of psychology (PsyD) or doctor of philosophy (PhD.)

How are services funded?
Services and supports provided by the Licking County Board of Developmental Disabilities are funded primarily by the Board’s two local tax levies.
